- The pre-departure ejection of a family from an international flight that boarded at Heathrow, London.
- The pre-departure ejection of a passenger and his spouse from a flight at LAX.
- A extraordinary event that occurred inflight wherein a man was killed by a group of fellow passengers.
- An injury that occurred when a woman fell from a boarding ladder at Burbank Airport when leaving the aircraft.
- An injury that occurred during boarding when one passenger hit another with her luggage.
- An injury that occurred during taxi prior to departure when an overhead bin spontaneously opened.
Mr. Ditchey held overall executive management responsibility for flight crew training, performance, and compliance with regulation at two major airlines. As such, he has a well-developed knowledge of the subject.

While certifying MGM Grand Air, Mr. Ditchey managed and successfully executed a successful, unprecedented and unusual slide evacuation of MGM's aircraft, which were unique. Not many airline executives get to experience this event, much less oversee and manage it.
